Thank you for enrolling in one of our Special Needs Plans (SNPs). Our SNPs feature special case management support and prevention programs that can help you remain as healthy and independent as possible.

We want to help you take advantage of these special benefits. That's why it is important that you understand the SNP enrollment process. Part of this process includes having your primary care physician confirm that you have been diagnosed with a cardiovascular condition, diabetes or chronic heart failure. The way your physician confirms this is by completing your verification form and returning it to us within thirty days of your enrollment.

We will do everything we can to ensure that we receive your verification form from your physician within this time period. We will contact your primary care physician and remind them to complete and return your verification form. We will also send you a reminder notice shortly after you enroll.

If we do not receive a completed verification form from your physician within thirty days, we are required to send you a letter of intent to disenroll from your plan. If you receive this letter, you will still have an additional thirty days to provide us with proof of eligibility (in this case, your completed verification form) to remain enrolled in your Special Needs Plan. If we do not receive this within thirty days, your disenrollment will be effective on the last day of your second month of enrollment.

We want you to receive the health care services you need. During this sixty day period, you will be able to access your health benefits.
